Could be any of the following:
1. Cylinder guide rings are worn/missing resulting in the wobble of the cylinder unit causing the sear not to be able to catch the piston.
2. Worn sears (most common problem)
3. Broken sears (perhaps it snapped or something)
4. Piston is worn down
